Intake Form Information: We collect information about you when you voluntarily complete a health history on the Site. For example, we collect physical, physiological, mental, economic, cultural, and social information that help the Company’s health coaches provide you with specific strategies and tips during your consultations.
Information we collect when you use the Site

Wix.com (“Wix”), which hosts the Site, collects information about you when you use our Services, including information tied to visits to the Site and taking certain actions as part of the Services.
Your use of the Services: Wix keeps track of certain information about you when you visit the Site and interact with any of our Services. Wix collects information about the pages within the Site you visit, the links you click on, the type, size and names of files you upload or download, and the search terms you use.

Our company is hosted on the Wix platform. Wix provides us with the online platform that allows us to provide our Services to you. Your data may be stored through Wix’s data storage, databases and the general Wix applications. They store your data on secure servers behind a firewall. Your rights as a Vermont resident: We do not share nonpublic personal financial information we collect about Vermont residents to non-affiliated third parties except for our everyday business purposes or as otherwise permitted by law. If you wish to revoke any authorization to share your nonpublic personal financial information, please contact us for assistance at info@joyvial.com, though we will no longer be able to provide you Services or access to the Site.
