Intake Form Information: We collect information about you when you voluntarily complete a health history on the Site. For example, we collect physical, physiological, mental, economic, cultural, and social information that help the Company’s health coaches provide you with specific strategies and tips during your consultations.

Wix.com (“Wix”), which hosts the Site, collects information about you when you use our Services, including information tied to visits to the Site and taking certain actions as part of the Services.
Your use of the Services: Wix keeps track of certain information about you when you visit the Site and interact with any of our Services.
